OTSI ACADEMY INTAKE — REQUIREMENTS CHECKLIST

Use this checklist to prepare your academy, team or programme before starting
the structured intake on otsi.cc/academy-intake/.

REQUIRED SECTIONS
------------------
[ ] Registered academy identity - legal name, registration number, address
[ ] Authorised contact - a named primary contact with authority to submit
[ ] Teams & programme information - age groups, number of teams, player count

UPLOAD / CONSENT
------------------
[ ] Player-group roster - your player group added to Step 3 of the intake
[ ] Supporting documents - the 7 documents requested at Step 5
[ ] Parent / player consent - guardian consent for every player under 18

BEFORE YOU BEGIN
------------------
[ ] You have authority to submit this intake on behalf of the academy.
[ ] All academy details (legal name, registration, address) are accurate.
[ ] Teams, age groups and number of players are clearly defined.
[ ] Named safeguarding / welfare contact person is identified.
[ ] You will obtain consent to share player information as required.
[ ] All required supporting documents are ready to upload.
